Keith Millen joined MK Dons on 23 January 2018 as assistant manager to Dan Micciche, bringing a wealth of experience from his coaching career, including spells working alongside Tony Pulis, Neil Warnock and Alan Pardew at Crystal Palace. Before arriving at Stadium MK, Millen had also managed Bristol City and briefly worked with Portsmouth.

His time as Micciche's assistant was short-lived. When Micciche left the club by mutual consent on 22 April 2018, with the Dons battling relegation from League One, Millen was placed in temporary charge for the final three games of the season.

Millen's caretaker spell produced one win, no draws and two defeats from three matches. Unfortunately, the results were not enough to save MK Dons from relegation to League Two.
